Our physician-directed, ACHC-accredited home health care team delivers skilled nursing, therapy, social work, and specialty programs directly into your home, so your loved one can recover, regain independence, and stay safely where they want to be.

What Is Home Health Care?

Home health care is a Medicare-covered benefit that brings skilled medical services directly into a patient’s home. It is physician-directed, clinically supervised, and built around a personalized plan of care with specific, measurable goals. It is not personal care or companionship.

A patient may qualify when they:

  • Are under the care of a physician who has ordered home health services
  • Are considered homebound, meaning leaving home requires a considerable and taxing effort
  • Need part-time or intermittent skilled care such as nursing, physical therapy, “occupational therapy” or speech-language pathology

Home health care supports patients working toward recovery and greater independence. It is designed to help patients get better, stay safer, and return to the activities that matter most to them.

Home Health Services

Our clinical services form the foundation of every patient’s plan of care. These are the skilled, physician-directed disciplines that address your loved one’s medical needs at home.

Services include:

  • Skilled Nursing – medication management, wound care, vital signs monitoring, infusion therapy, and patient education delivered by Registered Nurses and Licensed Practical Nurses
  • Physical Therapy – rebuilding strength, mobility, and balance after surgery, hospitalization, or illness
  • Occupational Therapy – restoring independence with daily activities including bathing, dressing, and meal preparation
  • Speech-Language Pathology – addressing challenges with communication, eating, drinking, and swallowing
  • Medical Social Work – navigating insurance, community resources, care transitions, and the emotional weight of serious illness
  • Home Health Aide Services – hands-on personal care and daily living support provided within a supervised, physician-approved plan of care

Every service is delivered by a clinician working as part of a coordinated team – not in isolation. Your physician remains at the center of the care plan throughout.

Home Health Care Team

Home health care is a team effort. At James River Home Health & Hospice, no single clinician carries the full weight of your loved one’s recovery. Registered nurses, physical and occupational therapists, speech-language pathologists, social workers, and home health aides work in close coordination with each other and with your physician – each bringing a different lens to the same goal.

What makes this coordination meaningful is communication. Your care team shares information, adjusts plans together, and ensures that every visit moves your loved one in the same direction. When one discipline identifies something outside their scope, they bring it to the team. Nothing falls through the gaps.

Our team has served patients across Virginia for more than a decade, with subspecialty experience in orthopedics, neurology, cardiology, pulmonology, and complex chronic disease management.

Home Health Specialty Programs

Core skilled services address what your loved one needs clinically. Our specialty programs go a step further – building condition-specific protocols, closer monitoring, and deeper physician coordination around particular recovery goals and chronic conditions.

These programs are designed with one outcome in mind: keeping patients out of the hospital. Through structured clinical oversight, early identification of warning signs, and patient education that actually sticks, our specialty programs have a demonstrated track record of reducing avoidable readmissions.

Programs include:

  • Orthopedics – structured rehabilitation following joint replacement, spinal surgery, and orthopedic injuries, built around your surgeon’s specific protocols
  • Respiratory Care – designed for patients managing COPD and moderate to chronic respiratory conditions, with a focus on improving breathing function and reducing hospitalization episodes
  • Cardiac Care – supporting patients recovering from cardiac events or managing heart failure, working alongside your cardiologist to monitor symptoms and reinforce long-term heart health
  • Neurological Diseases – specialized management strategies for patients living with stroke, Parkinson’s disease, ALS, multiple sclerosis, spinal cord injury, dementia, and Alzheimer’s disease
  • Complex Care – comprehensive support for patients with multifaceted conditions, including advanced chronic disease, wounds, cancer-related needs, and a history of frequent hospitalizations

Each program begins with a thorough clinical assessment and is adjusted continuously as the patient’s condition changes. Your physician is an active participant throughout.

How Home Health Care Begins

  1. A physician orders home health services. Your doctor, hospital discharge planner, or care coordinator initiates a referral. You are also welcome to call us directly at (855) 415-5744 to ask questions before a referral is placed.
  2. A clinician completes a home assessment. A Registered Nurse visits your home, reviews your loved one’s medical history and current needs, and works with the ordering physician to develop a personalized plan of care.
  3. Your care team begins. on your schedule. Visit frequency, disciplines involved, and specialty programs are determined by the plan of care. Our on-call clinical team is available 24 hours a day, 7 days a week, including nights, weekends, and holidays, should a concern arise between visits.

We work to begin care quickly following a referral. Our team coordinates closely with hospital discharge planners so that transitions home are as smooth and safe as possible.

Mr. Clay Dillon is an exception OT young Man.He is very informative with ideas to help you Succeed in your goal. He see any way to give you information towards Ramps , transportation, material that could be beneficial to your needs. He is kind and show that he care about his patient. He motivates them by giving them positive comments to build up their confidence in getting them back to the things they use to do. He has a good sense of humor by getting their minds off of the negative thoughts and turn them in to positive thoughts. He spend time with You, not rushing and treats you with dignity and respect.
