It goes beyond standard skilled nursing visits by adding a structured clinical framework defined protocols, systematic symptom monitoring, and a coordinated approach to patient education, built specifically around the patterns of pulmonary disease and the clinical factors that drive readmission.

- Pulmonary Exercise and Physical Rehabilitation Our physical therapists safely rebuild cardiovascular endurance and functional strength at an intensity appropriate to the patient’s current respiratory status.
- Early Warning Sign Recognition We educate patients and caregivers on the specific signs of a COPD exacerbation, when to act, and how to use a personalized action plan developed with the pulmonary physician.
- Pulmonary Physician Coordination Clinical findings, monitoring data, and patient progress are communicated regularly to the ordering physician and pulmonary specialist.
- Case Manager Coordination A dedicated Registered Nurse or physical therapist serves as your single point of contact throughout the program.

What to Expect When the Program Begins
- Before your first visit. Your Case Manager reviews your loved one’s medical history and your care team is briefed before anyone arrives at your home.
- At your initial home visit. A Registered Nurse or physical therapist conducts a comprehensive respiratory and functional assessment. The plan of care is developed in coordination with the ordering physician and pulmonary specialist.
- Throughout the program. Visit frequency is determined by clinical need and physician orders. Your Case Manager monitors the full picture and ensures nothing changes without the care team knowing.
- When the program concludes. Your care team coordinates a clear transition plan.
Our on-call clinical team is available 24 hours a day, 7 days a week, including nights, weekends, and holidays. Respiratory symptoms do not follow a business hours schedule, and our response to them does not either.
